• Link you with a general practice doctor local to your area. 
• Educate you on the Australian healthcare system and how it operates. 
• Work in collaboration with a translation service.

We work collaboratively with you, your family, your carers, and your broader systems of support to deliver the best outcomes for your future.  We partner with you to empower you to have full choice and control over how you would like to use your NDIS funding to achieve your goals to live your best life. Coast Transitional Support provides services to participants who are Agency Managed, Plan Managed or Self Managed.

The “C” in Coast Transitional Support is spiral-shaped to represent an opening fern leaf to symbolise new life, growth, strength, and peace, and this is our commitment to you.

Your Recovery Coach will empower you to recognise and take responsibility for your recovery and well-being. We will support you with your journey to:

· Develop and implement a recovery plan

· Identify and develop goals

· Build capacity and resilience through strong and respectful relationships

· Empower you to have full choice and control over your life

· Increase your social and economic participation

· Identify, plan, design, and coordinate your NDIS supports

· Access a wide range of support services targeted to you

 Coast Transitional Support recognises that every individual is unique. Every NDIS plan is different. And each individual’s needs differ. That’s why we work with you to be your partner on your NDIS journey, working collaboratively with parents, families, schools, support services, and health professionals.
